Guided by our core values of Passion, Respect, and Excellence , the Social Worker Assistant supports the Social Services department in delivering compassionate, compliant, and resident-centered care. Responsibilities for Social Worker Assistant: Assist the Social Services team in supporting residents with psychosocial, emotional, and practical needs. Help coordinate access to community resources, including healthcare services, welfare agencies, non-medical home care, and hospice services. Support implementation of departmental programs, goals, objectives, policies, and procedures. Maintain communication with residents, families, and interdisciplinary team members to support resident care planning. Assist with documentation, recordkeeping, and administrative tasks within the Social Services department. Support compliance with resident rights, safety standards, and regulatory requirements. Participate in maintaining a respectful, safe, and supportive environment for residents and staff. Perform other duties as assigned to support the Social Services department. Qualifications for Social Worker Assistant: College degree preferred. Long-term care experience required. Ability to make independent decisions when appropriate. Knowledge of practices, procedures, laws, regulations, and guidelines governing post-acute care facilities. Ability to implement and interpret departmental programs, goals, objectives, policies, and procedures. Competency in general social services functions, including patient rights, safety, and sanitation. Demonstrated integrity and professionalism in accordance with the company's Code of Conduct and completion of required compliance training. Knowledge of healthcare systems, welfare agencies, non-medical home care, hospice, and community social resources.
