Good Samaritan Health Center is seeking a dedicated RN, Minimum Data Set (MDS), to support our mission of providing comprehensive, holistic healthcare services to underserved communities. In this role, you will use your clinical expertise and attention to detail to ensure accurate and timely completion of MDS assessments, translating resident needs into meaningful, person-centered care plans that reflect our commitment to wellness, compassion, and community engagement. The RN, MDS will be responsible for coordinating and completing MDS 3.0 assessments and related Resident Assessment Instrument (RAI) processes for residents in our care, ensuring compliance with federal and state regulations and supporting appropriate reimbursement. You will collaborate closely with an interdisciplinary team—including physicians, advanced practice providers, social workers, therapists, dietitians, and nursing staff—to gather clinical information, validate documentation, and develop care plans that promote optimal health and functional outcomes. Your work will directly influence quality measures, clinical programs, and performance improvement initiatives that support Good Samaritan’s mission-driven habit of serving through health. Day-to-day, you will review charts and clinical documentation, interview residents and families as appropriate, and coordinate with staff to ensure that all MDS assessments are completed accurately and submitted within required time frames. You will monitor changes in resident status, initiate significant change assessments as needed, and update care plans to reflect evolving needs and goals. You will use your knowledge of Medicare and Medicaid regulations, ICD-10 coding associated with MDS elements, and reimbursement guidelines to help ensure accurate case-mix classification and alignment with regulatory requirements, while keeping the focus on resident-centered care rather than simply documentation. As part of our collaborative and community-focused team, you will help educate and support nursing and interdisciplinary staff on MDS requirements, documentation standards, and quality measure implications. You will participate in care conferences, quality improvement meetings, audits, and regulatory surveys, contributing clinical insight and data to strengthen resident outcomes and organizational performance. You will also assist in tracking and analyzing MDS-related data to identify trends, gaps, and opportunities for improvement in resident care and operational efficiency.
